Does it include health insurance or 401(k) deductions?

Not automatically. Employer benefits, pretax deductions and other payroll items can make your real paycheck different.

Why can state withholding be different from the estimate?

The tool uses simplified state tax rates instead of every bracket, deduction, credit and local payroll rule.
